The psoriasis market has become lucrative owing to dermatologists’ comfort and familiarity with prescribing biologics to patients with moderate to severe disease. The dominance of the tumor necrosis factor-alpha (TNF-α) inhibitors, primarily Amgen/Pfizer/Stiefel/Takeda’s Enbrel and Abbott/Eisai’s Humira, is being challenged by Janssen Biotech/Janssen-Cilag’s interleukin-12 and 23 (IL-12/23) inhibitor Stelara. Further intensifying the future competitive landscape in the United States, two agents in late-stage trials for psoriasis offer the advantage of oral formulation—Pfizer’s Jak-3 inhibitor tofacitinib and Celgene’s phosphodiesterase-4 inhibitor apremilast. In addition, topical drugs with novel mechanisms of action such as Incyte’s Jak-2 inhibitor ruxolitinib and Anacor’s AN-2728 are set to enter the psoriasis market over the next three years. However, the competitiveness of these agents and their likely positioning in the current treatment algorithm for psoriasis will be strongly impacted by cost as well as physician and payer perceptions of their risk-benefit profiles.

Decision Resources surveyed 104 dermatologists and 25 managed care organization (MCO) pharmacy and medical directors to gain better understanding of the factors shaping current and future treatment patterns as well as reimbursement trends in psoriasis, to reveal dermatologists’ perceptions of key marketed and later-stage oral and topical immunomodulators, and to evaluate opportunities for therapies targeting the TNF-α-refractory population.

This webinar, presented by Decision Resources Analyst Iva Holder, Ph.D., will answer the following questions:

  • How does Stelara’s formulary coverage compare with that of Enbrel and Humira and what coverage do surveyed MCOs expect for Stelara in one year’s time? How many surveyed payers require that patients fail TNF-α inhibitor treatment before they will reimburse Stelara?
  • According to surveyed dermatologists, which agents do patients typically fail before starting therapy with biologics? What cost control approaches are most frequently applied to biologics’ use for psoriasis by surveyed MCOs, and how do they vary by biologic? What are dermatologists’ perceptions regarding the stringency and impact of payer cost controls?
  • According to surveyed dermatologists, what percentage of patients treated with biologics currently are able to afford these agents solely due to copay assistance programs? What are surveyed dermatologists’ opinions regarding the effects of copay assistance programs?
  • What are surveyed MCOs’ expectations regarding tier positioning for the emerging oral agents tofacitinib and apremilast in various pricing scenarios? What step therapy restrictions are most likely to be imposed on tofacitinib? How many surveyed MCOs would consider requiring tofacitinib or apremilast as a step therapy before biologic treatment within three years of each agent’s approval for psoriasis?
  • Which patient groups do surveyed dermatologists believe will receive each emerging oral agent? What percentage of surveyed dermatologists will consider prescribing apremilast or tofacitinib before a TNF-α inhibitor if the emerging oral had lower, the same or higher copay than agents in this biologic drug class?
